Information for Immigration Medical Examinations for the United States of America
This is the information you need for your medical examination if you are applying for a visa for the United States of America.
Please bring the following:
Interview date appointment letter OR repeat medical request letter issued by the US Embassy
Original passport AND 1 photocopy
Two passport size photos
COVID 19 vaccine certificate (printed)
Other vaccination documents (eg. Hep B, TDP, MR, MMR, polio etc) for all applicants

Medical history: list of current medications, recent blood tests and x-rays, hospital notes, discharge summaries, doctors’ letters or prescriptions
For female applicants with children: Dates of birth (DOB) of all children (applicant or non applicant)
Additional Information
Plan to complete this medical examination at least two weeks before your interview.
No fasting required. Eat and drink as normal before the examination. Take all medications as prescribed by your doctor.
All completed medicals are submitted directly to the Consular Section of the US Embassy before the interview date.
Applicants under 18 years of age are required to present with a family member or guardian.
Vaccinations are compulsory and will be administered according to guidelines.
Please have the DHK number and interview date ready when booking an appointment.
Applicants who are aged under 18 years old, or have an intellectual or physical disability, will be allowed to present with a guardian. All other applicants are not allowed an accompanying family member or friend.
