Important information for your appointment

Bring the following items to your NMA medical appointment:

- Original passport OR
- Original Continuous Discharge Certificate (Seaman’s book)
Medical history: medication list, recent blood tests, x-rays, hospital notes, discharge summaries, doctors’ letters or prescriptions
Prescription glasses/contact lenses (avoid coloured contact lenses)

Certificates are issued on the next business day and must be collected in person.
